Prime Care Coordination
Prime Care is a network of people and services across Upstate New York State who work together to help people with intellectual and developmental disabilities.
Our job is to help each person and their family receive all the assistance they need.

Person Centered Services
Person Centered Services is a Care Coordination Organization serving over 17,000 people with Intellectual and Developmental Disabilities in 21 counties. Care Coordinators work on a "Life Plan" tailored to every one of an individual's needs.
We work with dozens of organizations and hundreds of providers, so people can meet all of their goals. New individuals to care coordination are accepted regardless of agency affiliation, and we work with 100% of the agencies in our service area.

OPWDD - Office for People with Developmental Disabilities
OPWDD offers an array of services and supports to help people with developmental disabilities live in the home of their choice; find employment and other meaningful activities in which to participate; build relationships in the community, and experience health and wellness. The various levels and combinations of services and supports mean that OPWDD can accommodate the strengths, needs, and preferences of virtually any person with developmental disabilities.

Erie 1 BOCES
There are 19 school districts that are components of Erie 1 BOCES. Those districts are able to enroll in a variety of instructional and non-instructional services offered by Erie1 BOCES. -
