Registration of students new to the Frontier Central School District can either print a hard copy or pick up a registration packet at the Registration Office. The registration office is open Monday-Friday from 7:30 a.m. - 3:30 p.m. An appointment is necessary to drop off and complete the registration. Call the office (716) 926-1734 to schedule the appointment.

Documentation Required for Registration of Students New to the District

●    Original birth certificate/passport
●    Proof of current immunizations. Must be fully immunized within 14 days from the start
date of your child to avoid exclusion from school; out of state registrants have 30 days.
●    Current Physical/NYS School Health Exam Form (link to print below)
●    Two current proofs of residency (issued within two (2) months must include name, address and date)
●    Legal documentation/custody/guardianship/divorce, if pertinent
●    Current IEP/504, if pertinent
●    Parent/Guardian photo identification

Acceptable proofs of residency are: (two are required: must include guardian name, address and current date)

●    Current mortgage, deed, closing statement, lease, rental agreement
●    Current utility bill/phone bill
●    Current Home/Renters/Auto insurance/Vehicle registration
●    Current pay stub/source of income
●    NYS driver's license issued within 2 years
