Instructions for Working Paper Applications
During COVID-19, please submit your completed and signed Working Papers application, along with verification of age ("copy" of birth certificate, permit or driver's license) and a current physical (if you have not already submitted it to the HS Nurses Office for this year). Physicals are only valid for a 12 month time frame. If your physical was done more than 1 year ago, a new physical must also be submitted. All items can either be (1) dropped off at the HS Main Office; (2) faxed to the HS Main Office at (716) 646-2195 or (3) emailed to firstname.lastname@example.org. Once working papers are validated and completed, they will be available for pick up in the Main Office at the High School (or will be mailed to you if so requested). Once received by you, they need to be signed prior to submitting them to your employer.

Student/Parent should complete Part I, and your application must include a parent’s signature.
NOTE: PER A DIRECTIVE FROM THE GOVERNOR OF NYS, EFFECTIVE JANUARY 2010, STUDENT SOCIAL SECURITY NUMBERS ARE NO LONGER REQUIRED ON THE APPLICATION OR WORKING PAPERS.
Student/Parent will need to either bring/mail/email (see above) the “completed application," an original or copy of either: their birth certificate OR passport OR valid driver’s license, AND a valid physical (**) to the Main Office at Frontier High School.
** A valid physical is either (1) a sheet obtained from the school nurse if the student has had a physical done at the high school within the past 12 months, OR (2) a note from the student’s personal physician stating the date of their physical (must have been done within the past 12 months) and a verification statement that the student is fit and able to work without restrictions.
